Home Support

Following referral, a care coordinator undertakes an initial home visit to ascertain eligibility and level of services required:

Home Care is practical household assistance, in duties such as vacuuming, cleaning bathroom/toilet, washing and ironing, changing and making beds and some meal preparation.

Personal Care offers assistance with showering, bathing, dressing and undressing, toileting, grooming, monitoring of self-medication, transport, mobility assistance and eating/drinking assistance.

Respite Care assists frail aged and people with disabilities to remain at home and in the community by providing carers, including parents, spouses and family members, with a break from their caring responsibilities. The service can be provided in the client’s home, in the community or a combination of both depending on the needs of the client and carer. It is available on weekends, after hours including weekends, overnight and in emergency situations.

Planned Activity Groups provide activities directed at enhancing the skills required for daily living and providing physical, intellectual, emotional and social stimulation. They also provide opportunities for social interaction as well as respite and support for carers. Groups are conducted regularly across most communities. A weekly program for younger adults with disabilities is conducted in Sale, with a monthly program operating on a Saturday for teenagers with disabilities.

Delivered Meals assist frail older people and people with disabilities (and their carers) to live independently at home through provision of pre-prepared meals.

Community Transport assists in the transport needs of elderly, frail aged or disabled people. A pool of volunteer drivers takes eligible clients to medical appointments both regionally and to Melbourne.

Home Maintenance Services provides a small range of services, supporting clients to live independently at home. For example, fitting grab rails to bathroom, toilet, showers and doorways; fitting hand rails to outside steps; and construction of ramps.
