A Home Care Package is funding from the Commonwealth Government to assist with services and supports to keep you living at home independently.
To apply for a Home Care Package, visit the My Aged Care website and fill in an online application form, or alternatively call them on 1800 200 422. An assessor will contact you to determine your needs and eligibility. If deemed eligible you will be placed in a waitlist for a Home Care Package.
Once you receive confirmation of your Home Care Package being assigned you will need to contact a service provider to assist with managing your Package.
There are four levels of funding available ranging from Level 1 – Basic Care to Level 4 – High Care.
Central Gippsland Health is an approved Home Care Package Provider and works with consumers within the Wellington Shire to develop a Care Plan for the services required to reflect your needs. It’s all about your choices.
We have a team of case managers who will work with you and get to know you so we can provide services that are tailored to your needs.
