HOW TO ACCESS THIS CLINIC

Referral from your GP
A referral from your GP is required for all patients wanting access to the clinic. Please include the below Maternity Referral Form.

A copy of all available pathology and ultrasound reports should be included.
For Antenatal Patients it is recommended that the below antenatal screening tests be included.

FBE
Blood Group and Antibody Screen
Rubella
Hepatitis B
Hepatitis C
HIV
Syphilis
MSU
Downs Screening
VITD (for high risk group)

WHAT TO BRING
Patients must provide their Medicare card at all appointments.
Antenatal Patients are required to provide both their Medicare Card and Antenatal Care Record at all appointments.

Gynaecology
Antenatal Pre Admission Service (ANPAS) All patients birthing at CGH, or those birthing elsewhere that may present at CGH during their antenatal period, are required to attend ANPAS appointments (between 16-20 weeks gestation) at no cost. During this appointment, the midwife will provide essential pregnancy information to you and your family, including information about having a baby at CGH.
The midwife also obtains information from you to guide care and assist in planning additional services to meet your individual needs.
