Please include the below Maternity Referral Form: Download the Maternity Referral Form A copy of all available pathology and ultrasound reports should be included. For Antenatal Patients it is recommended that the below antenatal screening tests be included: FBE Blood Group and Antibody Screen Rubella Hepatitis B Hepatitis C HIV Syphilis MSU Downs Screening VITD (for high risk group) WHAT TO BRING Patients must provide their Medicare card at all appointments. Antenatal Patients are required to provide both their Medicare Card and Antenatal Care Record at all appointments. SENIOR MANAGEMENT TEAM MS MANDY PUSMUCANS, INTERIM CHIEF EXECUTIVE OFFICER Mandy is a registered nurse with additional qualifications in rural health and business management. Following a working life that commenced in Gippsland, Mandy worked in Melbourne hospitals, rural Victoria and then on to remote Australia, which included living and working in both remote Aboriginal communities in Central Australia and the main centre of Alice Springs. Mandy’s work has focused on social justice and equity of access for rural and remote communities, with particular focus on Aboriginal Australians. Mandy has been both an executive officer and then Board member of the Council of Remote Area Nurses Australia, a participant on the Council of the National Rural Health Alliance, and a member of the Royal College of Nursing Australia. DR TRACY MCCONNELL-HENRY, ACTING DIRECTOR OF NURSING Tracy is a registered nurse with a clinical background in critical care and organ donation. Her dedication to critical care nursing has seen her hold posts as clinical educator and resuscitation coordinator before being recruited to Monash University to develop the curriculum and coordinate the Critical Care program, delivering it across south east Victoria, to both regional and tertiary health services, as well as adaptions in the Masters programs in South Africa, Hong Kong, Malaysia and PNG. In addition to her teaching commitments Tracy immersed herself in research, publishing and presenting, primarily in the areas of phenomenology, rural nursing issues and decision making in the deteriorating patient. Additionally, she has been a peer reviewer on several international professional journals and a higher degree examiner. Tracy has a Bachelor of Nursing, Graduate Diploma of Nursing (Critical Care), Masters of Health Science (Nursing Education) and PhD by full publication. She has also previously been an Adjunct Research Fellow at Monash University, Tracy is driven to continue to strive for excellence in nursing care, motivated by the desire to have CGHS a place staff are proud to work and somewhere we choose to have our families and community cared for. DR DIVYANSHU DUA, DIRECTOR MEDICAL SERVICES (FRACP, FAGE) Dr Divyanshu ‘Divy’ Dua is Director of Medical Services at the Central Gippsland Health Service. Additionally, he is a visiting medical oncologist at the Latrobe Regional Health Service. He also holds the clinical dean position with the Monash Rural, Sale campus. A consultant medical oncologist and Fellow of the Royal Australasian College of Physicians (RACP). Dr Dua trained in Internal medicine and Medical oncology in Australia as part of FRACP before completing a clinical fellowship in drug development, early phase trials and thoracic malignancies at the Guy’s Hospital, London (UK), and the Sarah Cannon Research Institute (UK). Thoracic malignancies – including lung cancer, mesothelioma and thymoma – continue to be of special clinical interest to Dr Dua. He also has particular interest in the treatment of Genitourinary malignancies – especially prostate cancer, malignant melanoma, CNS tumors and breast cancer. Dr Dua has also involved in clinical research in the field of geriatric oncology “cancer in the older people” services as part of his role as medical oncologist. Dr Dua has published several articles in peer reviewed international journals. He is actively involved in teaching and research through his current affiliation with the Monash rural medical school. His past academic affiliations include the Australian National University, Canberra. University of Adelaide, Flinders University, Monash Rural school of Medicine in Australia and King’s College London. MS KELLI MITCHENER, DIRECTOR QUALITY & LEARNING SERVICES Kelli is a registered nurse with a passion for quality, risk and governance. She began her nursing career in Melbourne spending 13 years in Intensive Care at Austin Health. During that time Kelli also gained experience through project roles; in clinical governance, resuscitation, quality improvement, rapid response systems and service development. Kelli moved into a variety of roles related to trauma, simulation and exercise training, and process redesign including time at the Office of the Emergency Services Commissioner, the Department of Health and Human Services and NSW Health. Kelli is a specialist reservist in the Royal Australian Air Force and deployed to Afghanistan in active operations. Kelli has a Bachelor of Science (Nursing), a Diploma of Critical Care Nursing, a Master of Enterprise, and is a member of the Australian College of Nursing. MS MEGAN TYLER, DIRECTOR AGED CARE SERVICES Megan is passionate about providing the highest standard of care and quality of life to older people. She has worked across many areas of the health sector including the acute hospital setting, community care, specialist care and subacute care. Megan holds a Bachelor of Nursing from Monash University and prior to her role with Central Gippsland Health, was the General Manager at Lyrebird Village - Respect Aged Care in Drouin where she managed multiple teams and up to 118 aged care beds. MR ROBERT STRICKLAND, DIRECTOR CORPORATE SERVICES Robert has more than 20 experience in corporate services and senior executive roles in government, not for profit and private organisations across a broad range of industries including, primary health care, power generation, mining, construction, and port logistics industries. Robert is a Certified Practicing Accountant with CPA Australia and a Graduate of the Australian Institute of Company Directors as well as holding a Bachelor of Business. OUR REGION Central Gippsland Health is based in the diverse Gippsland region in the south east section of Victoria. It serves most of the Wellington Shire, excluding Yarram in the south of the Shire, which has its own health service. With services based in Sale (population approximately 14,000), Maffra (5,000) and the towns of Heyfield (2,000) and Rosedale (1,600), Central Gippsland Health is the major provider of health and community services in Wellington Shire. Total population of the Shire is around 42,000. The area is characterised by its strong agricultural, timber, aviation – both private and Defence Force – and tourism sectors. The lifestyle benefits for people living in the area are particularly strong, with the Gippsland Lakes, Australia’s largest inland waterway, and the Ninety Mile Beach in the south and east. HOME SUPPORT Central Gippsland Health provide a range of supports in your own home to assist you to continue living independently at home. These services are funded through a variety of programs such as the Commonwealth Home Support Program (CHSP), Home and Community Care Program for Younger People (HACC), Short Term Restorative Care program (STRC), Transition Care Program (TCP), Home Care Package (HCP) which from 1 July 2025 will transition to Support At Home, and National Disability Insurance Scheme (NDIS). Home Support services include: PERSONAL CARE This service provides people with support in activities of daily living that help them maintain appropriate standards of hygiene and grooming. These activities include support with: Eating Bathing Toileting Dressing Grooming getting in and out of bed moving about the house assistance with self-administration of medicine DOMESTIC ASSISTANCE This service provides people with assistance with domestic chores to maintain their capacity to manage everyday activities in a safe, secure and healthy home environment. This service is not a replacement for all domestic tasks but is designed to help people with the tasks they struggle with. These activities may include: Changing bed linens Vacuuming and mopping floors Cleaning one bathroom per visit Wiping cupboards and benches Date checking food items Minor food preparation Hanging / folding laundry Ironing Dusting Shopping assistance FLEXIBLE RESPITE This service supports and maintains care relationships between carers and care recipients, through providing good quality respite care for people so that carers can take a break. This includes: In-home day respite One on one support to assist someone in their own home to enable their carer to take a break. Community access respite One on one support to assist people to access their community undertaking activities they are interested in or need support to undertake, while giving their carer a break from caring responsibilities. DELIVERED MEALS This service prepares and delivers meals to people in their homes. The meals consist of a soup, main meal portion, a dessert and fruit. Meals can either be delivered daily as a chilled meal or weekly/fortnightly as frozen meals. Chilled Sale meals are delivered daily in the morning between 8am-9am or 11.30am-1pm Chilled Maffra meals are delivered daily between 11.30am and 1pm Frozen meals are delivered: weekly on Wednesdays to Sale, Maffra, Stratford weekly on Tuesdays to Heyfield and Rosedale fortnightly on Tuesdays to Gormandale fortnightly on Wednesdays to Loch Sport SOCIAL SUPORT GROUPS This service assists people to participate in community life and feel socially included through structured, group-based activities that develop, maintain or support independent living and social interaction. These activities include: group-based activities held in or from a facility/centre (e.g. pre-set or individually tailored activities promoting physical activity, cognitive stimulation and emotional wellbeing) group excursions conducted by centre staff but held away from the centre VOLUNTEER TRANSPORT This service aims to assist you and your carer if you are unable to transport yourself for medical appointments in regional Gippsland and Melbourne. Drivers are not allowed to lift a wheelchair into the boot or assist you in walking into the appointment. You can have a companion with you when you travel. SPECIALIST OUTPATIENT CLINICS DIALYSIS The Dialysis Unit, a satellite centre for St Vincent’s Hospital, provides haemodialysis for people in Central and East Gippsland as well as visitors to the region. There is an emphasis on education and training of patients and staff, and involvement in community health awareness programs. To enquire about regular or holiday dialysis, contact staff on 03 5143 8170 . ONCOLOGY The Oncology Department operates within the hospital to provide support and administer chemotherapy treatments to patients. It works in close association with the Medical Unit, Central Gippsland Health Palliative Care Services, and the Gippsland Regional Integrated Cancer Service. A palliative care facility located in the Medical Unit is available to patients who have been diagnosed with a terminal illness, where family and carers who wish to continue caring for the patient receive appropriate nursing support. PHARMACY The Pharmacy Department is staffed by pharmacists, pre-registration pharmacists and pharmacy technicians. The department is open from 8.30am until 5pm Monday to Friday, with an on-call pharmacist providing after hours and weekend services. The wide range of services offered include: Imprest drug distribution Clinical ward pharmacy services IV additives and Parental Nutrition Cytotoxic drug preparations e.g. chemotherapy Radiopharmaceutical preparations Drug information Drug trials Outpatient dispensing Medical and nursing staff education Support pharmacy services to smaller hospitals and private hostels PATHOLOGY (PROVIDED BY DOREVITCH PATHOLOGY) The Pathology Department provides a range of diagnostic tests to both inpatient and community based patients. The department is open for outpatients from 8am – 5pm, Monday to Friday. An after hours ‘on-call’ service is provided to the hospital.
