NDIS WHAT DOES THE NDIS DO? PROVIDING FUNDING TO ELIGIBLE PEOPLE BASED ON THEIR INDIVIDUAL NEEDS The National Disability Insurance Scheme (NDIS) provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life. Every NDIS participant will have an individual plan that lists their goals and the funding they have received. NDIS participants use their funding to purchase supports and services that will help them achieve their goals. Everyone has different goals, but they could include things like getting and keeping a job, making friends or participating in a local community activity. NDIS participants control the support they receive, when they receive it, and who provides it. WHO IS ELIGIBLE? 1. Are you under 65 years old? To access the NDIS you must be younger than 65 years old at the time you apply. 2. Do you have Australian residency? To access the NDIS you must live in Australia and be: an Australian citizen, OR a permanent resident, OR hold a Protected Special Category Visa 3. Do you usually need support from another person or use special equipment to complete everyday tasks because of a permanent and significant disability? 4. To access the NDIS you need to have a permanent and significant disability that stops you from doing everyday things by yourself. This includes disabilities that are episodic such as people who have a significant and lifelong psychosocial disability. 5. Do you need some supports now to reduce your support needs in the future? To access the NDIS to receive early intervention supports you must: have a permanent disability, or be a child younger than 9 years with a significant developmental delay requiring a certain level of intervention, and There needs to be evidence that getting support now will help you by: reducing the impacts of your disability or developmental delay and building your skills and independence. WHAT SERVICES WILL CGH PROVIDE UNDER NDIS? CGH has registered with the NDIS to provide the following services: Early Childhood Supports – this includes all Allied Health services for children younger than 9 years. Therapeutic Supports – this includes all Allied Health services for people between 9 and 65 years. Home modification –Occupational Therapy Assistive Products -Household Task – Occupational Therapy Communications& Information Equipment – Speech Pathologist Exercise Physiology/ Personal Training – Exercise Physiologist Personal Mobility Equipment – Occupational Therapy We can still provide other services to the participant in addition to the ones we are registered to provide under NDIS. RESIDENTIAL INREACH PROGRAM Better@Home services aim to deliver more specialised care closer to home and in the persons setting of choice. As one of the Better@Home services, Residential In Reach provides specialist assessment services by a Nurse Consultant, to people living in Residential Aged Care Facilities, which in turn, prevents avoidable presentations to the Hospital Emergency Department. Residential In Reach Nurse Consultants may visit and provide this specialist care; which is done in partnership with the Facility staff and the persons own General Practitioner. It is a team approach to supporting care in that persons’ setting of choice – Better@Home! If the person requires hospital admission, this decision is made with the person and their care team. Residential In Reach is NOT an emergency service. SERVICES WE PROVIDE Comprehensive Geriatric Assessment for people at risk of hospitalisation. Review and management plans for acute illness. Falls assessment and management. Advanced Care Planning. Palliative care advice and support. Continence care and advice including catheters, bowel management and bladder scans. Wound management, suture and skin advice. Intravenous access and antibiotics. PICC and PORT care and management. Dehydration management. Heart failure and chronic respiratory management. Doppler ultrasound. Portable electrocardiogram may be available. Education and staff upskilling as negotiated. HOSPITAL CARE Patients admitted to Maffra Hospital will stay in either a single or double room. Upon admission to Maffra Hospital, a detailed health assessment will be undertaken when you arrive by members of your health care team. You will be cared for by your doctor, qualified nurses, and members of the allied health team. Maffra Hospital also offers Palliative Care and Geriatric Evaluation Management services.

PROCUREMENT PROCUREMENT The Victorian Government Purchasing Board (VGPB) reformed procurement guidelines in 2014 for all government departments and public sector agencies. Following this process, Health Purchasing Victoria (HPV) aligned its procurement policies to the VGPB reforms. This reform is designed to ensure probity, planning, purchase category analysis as well as clear accountability and transparency underpin procurement models. In 2015 HPV mandated that all health services need to comply with HPV purchasing policies and that health services must transition to the HPV procurement policies by 30 June 2016. Central Gippsland Health has established a Procurement Framework that provides guidelines and procedures for conducting any procurement activity. The Framework and its associated procurement policy and procedures offer a strategy that ensures a coordinated approach to procurement at all levels of CGH. The Framework ensures strategic planning, equity of supply, opportunity for local businesses and demonstrated value for money for all of CGH’s procurement activities. TERMS AND CONDITIONS ACTIVITY PLAN SUMMARY COMPLAINTS PROCESS A procurement complaint is an issue or concern expressed by a supplier in relation to the process and probity applied by CGH in the procurement activity for a good or service. A complainant can refer a complaint about goods and services to anyone within CGH. All formal complaints are to be referred to the Chief Procurement Officer (CPO) for review, investigation (if warranted) and response. The complainant is required to provide details of the concerned dispute or complaint being lodged, including: A clear written statement of the perceived unsatisfactory approach and/or actions in the procurement process Copies of or references to information to support the complaint A statement regarding the desired outcome from the complaint process If further correspondence or information is required, the supplier will be given no less than 15 working days to respond to any communication from CGH unless the matter is urgent Complaints are to be lodged in writing or email to: Chief Procurement Officer Email: cpo@cghs.com.au Central Gippsland Health 155 Guthridge Parade Sale, Victoria 3850 OUR PROCUREMENT COMPLAINTS OFFICER Our Chief Procurement Officer will receive and review the complaint, then assign a dedicated procurement complaints officer. The Procurement Complaints Officer will be independent of the procurement process and the subject matter of the complaint. Upon receipt of the complaint, the officer will: Acknowledge receipt of the complaint within 5 working days of receiving it. This acknowledgement will contain the contact name of the investigating officer, the proposed action and the associated timeframes If additional information or clarification of complaint is required, CGH will advise the supplier of this. CGH and the complainant will come to an agreed extension of time The complainant should be advised in writing of the process undertaken by CGH, the outcome of the investigation, usually within 30 working days of receiving all written correspondence relating to a complaint. This correspondence should also include escalation process if the complainant is unsatisfied with the outcome. UNRESOLVED COMPLAINTS If the complainant is unsatisfied with the outcome from CGH, the unresolved complaint that specifically relate to Health Purchasing Victoria (HPV) contracts can be directed onto the HPV Board for review. All other complaints can be referred the Victorian Government Purchasing Board (VGPB). A complainant can refer a complaint to the HPV Board/VGPB for review if not satisfied with the findings and actions of CGH. This could be related to the management of the complaint or the application of procurement policy and procedures. CGH is to inform the HPV Board/VGPB within five working days of any complaint that could not be resolved to the satisfaction of both parties. HEYFIELD HOSPITAL BOARD OF MANAGEMENT RAELENE HANRATTY, CHAIR Raelene has been a member of the Australian Institute of Company Directors since July 2015. Raelene and her husband Dennis dairy farm at Upper Maffra West where they have raised 7 children. Raelene is dedicated to the local community and has held many executive committee positions on various organisations. She is currently the Deputy Chair of Southern Rural Water’s ‘Macalister Customer Consultative Committee’, the secretary of the Heyfield Tennis Club, and the treasurer of the Nambrok Newry Junior Football Club. Raelene is committed to ensuring that Heyfield Hospital and Laurina Lodge provide quality services to the Heyfield and District community. Appointed to the Board in 2015. ANNE WOOTON, VICE CHAIR Ann has been in executive and general management since 1998. Ann’s career has been in professional services, firstly in Australian law firms in knowledge and operational management, moving to the compliance and risk management services industry in executive management roles. Her most recent executive role was in the Property services industry and as an executive director of a global risk management company. She has worked with clients from major Australian banks to Fortune 500 organisations globally. Ann was employed by SAI Global from 2000 – 2017, an ASX-200 public company until acquired by private equity and delisted in December 2016. For the majority of this time, Ann was in executive management positions in Australia and the USA. Firstly, as General Manager of the APAC Compliance Division, followed by President of the Americas Compliance Division. Ann was most recently the Director of the Property Division achieving significant year-on-year growth, with annual revenue in excess of $180m. Ann was also an Executive Director and member of EXCO of this global organisation. Ann has recently retired to Gippsland in country Victoria and is on a steep learning curve in developing a very different skills set! PETER SKEELS Peter has over 36 years’ experience in the provision of financial, commercial, and strategic advice within the utility and service provision industries. His past roles have included Financial Planning and Strategy, Chief Financial Officer and General Manager, Customer Services. Peter is currently employed with Gippsland Water where his substantive role is General Manager, Operations. In recent years, Peter has been extensively involved in the justification, financing, construction and delivery of the Gippsland Water Factory project, a $230m advanced wastewater treatment and recycling project located near Morwell, Victoria. Peter holds a Bachelor of Business, Monash University with majors in Accounting and Economics and is a Fellow of the Australian Society of Certified Practicing Accountants (FCPA). He has held the following voluntary appointments, Past Chairman, Gippsland Branch CPA 1998 – 2004, Director and Deputy Chairman, Gippsland Grammar School, Sale 2004 – 2013 and Director Heyfield Hospital Incorporated 2014 – present. Peter’s primary recreational interests revolve around scuba diving where he is recognised by the Professional Association of Diving Instructors as a Master Scuba Diver Trainer and with Technical Diving International as an Instructor. Appointed to the Board in 2014. JOHN SUNDERANN John was educated at Heyfield primary school and Gippsland Grammar, spending a semester at the University of Illinois in year 12. On finishing school, John has worked on the family farm at Heyfield running angus cattle stud, a poll Dorset sheep stud and a dairy farm. John has been President of the Catholic College (Sale) school Board, Secretary/Treasurer of the Seaton Fire Brigade, Secretary and Treasurer of the Heyfield Golf Club and a member of the Macalister Consultative Committee for Irrigators. Appointed to the Board in 2014. SARAH CHRISTIAN Sarah grew up in Heyfield and completed a Bachelor of Science at Melbourne University. After travelling overseas and working at the Great Ormond Street Hospital for Sick Children in London UK, she returned to Australia to complete a Bachelor of Medicine and Surgery at Flinders University in Adelaide. Sarah completed her specialist training with the Royal Australian College of General Practitioners in 2014. She enjoys teaching and also works part time for Eastern Victoria GP Training as a Medical Educator and Training Advisor. She is passionate about addressing the rural shortage of Doctors and medical services. Sarah has three beautiful children and loves spending time with her extended family. Appointed to the Board in 2017. LIZ DRIFFIELD Appointed to the Board in 2023. GLEN WILLIAMSON (OBSERVER) Since 2019, Glen has been a Commercial Executive looking after 7 small global businesses in the role of Chief Operations Officer for a private entrepreneur with a combined turnover of $50 million dollars, covering all facets of operations such as Risk Strategy, Mergers and Acquisitions and Financial Governance in industries as varied as Technology Hardware, Digital Software Services, Motor Sport and Meal Replacement Shakes across a multitude of geographies. Prior to that, Glen was a Global Procurement professional for more than 20 years. Glen brings experience across the Retail, Pharmacy Distribution, Not for Profit, Finance, IT, Facilities Management and Telecommunications Technology industries ns many times along the journey. Glen also has an interest in Mental Health and is currently studying to gain a Diploma of Counselling with the Australian Institute of Professional Counsellors. Appointed as an observer in 2024. DAVID GRAHAM (OBSERVER) David was educated at Heyfield Primary and then Gippsland Grammar for his secondary years. After finishing year 12, he worked with a Timber Import/Export company in Melbourne for 4 years before returning to work in the family business. David has been Sole Director of Vern Graham Pty Ltd since July 2000. He served as a Board Member of Heyfield & District Community Financial Services for 11 years, including 3 years as Treasurer. Being a keen sportsman, David has served on committees of numerous local clubs and community organisations. Appointed as an observer in 2024. MARY MANESCU (OBSERVER) Mary is the CEO of Omeo District Health (ODH). Prior to her appointment at ODH, she held a range of clinical nursing and senior leadership roles in the private and public health sector. She also worked extensively for the Department of Health (Victoria) in various areas including regulation, major capital works, performance and information management. Mary holds a Master of Health Services Management and is a graduate of the Australian Institute of Company Directors. Beyond her substantive role, Mary is an accreditation surveyor with the Australian Council on Healthcare Standards Appointed as an observer in 2024.
