Home Support

Central Gippsland Health provide a range of supports in your own home to assist you to continue living independently at home.  These services are funded through a variety of programs such as the Commonwealth Home Support Program (CHSP), Home and Community Care Program for Younger People (HACC), Short Term Restorative Care program (STRC), Transition Care Program (TCP), Home Care Package (HCP) and National Disability Insurance Scheme (NDIS)

Home Support services include:

Personal Care

This service provides people with support in activities of daily living that help them maintain appropriate standards of hygiene and grooming. These activities include support with:

  • Eating
  • Bathing
  • Toileting
  • Dressing
  • Grooming
  • getting in and out of bed
  • moving about the house
  • assistance with self-administration of medicine

Domestic Assistance

This service provides people with assistance with domestic chores to maintain their capacity to manage everyday activities in a safe, secure and healthy home environment. This service is not a replacement for all domestic tasks but is designed to help people with the tasks they struggle with.  These activities may include:

  • Changing bed linens
  • Vacuuming and mopping floors
  • Cleaning one bathroom per visit
  • Wiping cupboards and benches
  • Date checking food items
  • Minor food preparation
  • Hanging / folding laundry
  • Ironing
  • Dusting

Flexible Respite

This service supports and maintains care relationships between carers and care recipients, through providing good quality respite care for people so that carers can take a break. This includes:

  • In-home day respite
    One on one support to assist someone in their own home to enable their carer to take a break.
  • Community access respite
    One on one support to assist people to access their community undertaking activities they are interested in or need support to undertake, while giving their carer a break from caring responsibilities.

Delivered Meals

This service prepares and delivers meals to people in their homes.  The meals consist of a soup, main meal portion, a dessert and fruit.  Meals can either be delivered daily as a chilled meal or weekly/fortnightly as frozen meals.

  • Chilled Sale meals are delivered daily in the morning between 8am-9am or 11.30am-1pm
  • Chilled Maffra meals are delivered daily between 11.30am and 1pm
  • Frozen meals are delivered
    weekly on Wednesdays to Sale, Maffra, Stratford
    weekly on Tuesdays to Heyfield and Rosedale
    fortnightly on Tuesdays to Gormandale
    fortnightly on Wednesdays to Loch Sport

Social Support Groups

This service assists people to participate in community life and feel socially included through structured, group-based activities that develop, maintain or support independent living and social interaction.  These activities include:

  • group-based activities held in or from a facility/centre (e.g. pre-set or individually tailored activities promoting physical activity, cognitive stimulation and emotional wellbeing)
  • group excursions conducted by centre staff but held away from the centre

Volunteer Transport

This service aims to assist you and your carer if you are unable to transport yourself for medical appointments in regional Gippsland and Melbourne. Drivers are not allowed to lift a wheelchair into the boot or assist you in walking into the appointment. You can have a companion with you when you travel.
