Regional Assessment Services

Welcome to the Commonwealth Home Support Programme (CHSP), Australia’s national funding initiative designed to assist individuals aged over 65 (or over 50 for Indigenous Australians) in maintaining their independence within their homes and communities.

To access the valuable services offered by CHSP, the first step is to register with My Aged Care and schedule an assessment through the Regional Assessment Services (RAS).

How to Register with My Aged Care: Simply call My Aged Care at 1800 200 422, allowing at least 10 minutes for the process. Please have your Medicare card ready to respond to the questions asked during registration. https://www.myagedcare.gov.au/

If you are a caregiver or family member assisting someone with My Aged Care, ensure you have consent to speak on their behalf. Refer to the My Aged Care Portal – Talking to My Aged Care for guidance. https://www.myagedcare.gov.au/arranging-someone-support-you

My Aged Care Referral to RAS  Assessment: Once registered, My Aged Care will refer you for a Home Support assessment with a Regional Assessment Service (RAS).

Your RAS Assessment: An Intake Officer from our team will reach out to you to schedule a convenient in-home appointment. During the Assessment: Our team member will discuss various aspects, including:

Your management of daily tasks and identified challenges

Your goals and aspirations, focusing on what matters most to you

Health and well-being, addressing any health concerns

Evaluation of your support system and community involvement to understand your needs.

Home Support Plan: Following the assessment, the assigned officer will develop a customized home support plan tailored to your individual needs.

Included in Your Support Plan: The support plan outlines strategies to help you achieve your goals and maintain independence.

Additional Assistance: If the RAS assessor determines that your needs go beyond what CHSP can provide, they will arrange for a more comprehensive assessment by an Aged Care Assessment Team (ACAT).
